Medication mistakes are one of the top reasons older adults end up back in the hospital. A missed dose. A doubled dose. The wrong pill at the wrong time. The fix is supervision and consistency.

AnchorPoint provides three levels of medication services under our comprehensive license. Most agencies in Oregon can only offer reminders. We can do reminders, hands-on assistance, and full medication administration under RN supervision, with physician orders.

What level of help your loved one needs depends on their cognitive and physical ability. We assess that during the free consultation using the official Oregon Health Authority self-direct evaluation tool, and we re-evaluate every 90 days as conditions change.

Everything Our Caregivers Provide

Medication Reminders

The caregiver reminds the client to take their medications. The client handles and self-administers.

Medication Assistance

The caregiver hands the client their medication, opens containers, pours water. The client self-administers.

Medication Administration

The caregiver administers medications directly under RN supervision and documented physician orders.

Pill Box Setup

Weekly or monthly pill organizer setup using current prescriptions and pharmacy labels.

Pharmacy Coordination

Refill tracking, pharmacy pickups, and communication with the prescribing physician.

MAR Documentation

Medication Administration Records kept for every dose, available for physician and family review.

Common Situations Where Medication Management Makes a Difference

From First Call to First Visit

Free assessment

Our RN reviews the current medication list and observes how the client manages medications today.

Self-direct evaluation

If the client can self-direct (knows their meds, doses, times, and reasons), we provide assistance. If not, we administer with physician orders.

Pharmacy coordination

We work with the client's pharmacy for refills, packaging, and any changes to the regimen.

Daily documentation

Every dose is recorded. The RN reviews records and reassesses every 90 days.

Questions Families Ask About Medication Management

What's the difference between reminding, assistance, and administration?

Reminders mean we tell the client it's time to take their pills and the client does everything else. Assistance means we help with containers and water but the client self-administers. Administration means we give the medication directly, which requires physician orders and RN oversight.

Can you give insulin injections?

Yes. We administer insulin and other injectable medications under formal RN delegation with physician orders, for clients with stable and predictable conditions.

What if the medication list changes?

We update the MAR immediately when a physician changes the regimen, and the RN reviews each change. Pharmacy coordination is part of the service.

Does Medicaid cover medication services?

Yes, for clients with assessed need. Oregon Health Plan covers medication services through in-home care agencies. We help families navigate eligibility during the assessment.
